Daytime TV described Viki’s stroke storyline as one of Erika Slezak’s most challenging assignments, with Erika calling it her most difficult storyline to that point.

After being elected mayor of Llanview, Viki took on organized crime and drug trafficking in the city, putting her directly in the path of Carlo Hesser and his son, Johnny Dee. On Johnny’s orders, corrupt police commissioner Harding Richards attempted to assassinate Viki during a televised appearance. Viki survived the gunshot and extensive surgery, but while beginning to recover she suffered a devastating stroke.

The stroke left Viki paralyzed on her right side and unable to walk or speak. Returning home in a wheelchair, she began the slow and frustrating work of rehabilitation with therapist Sheila Price. The story followed not only Viki’s physical struggle to regain movement and speech, but also the emotional reality of suddenly being unable to communicate or perform the simplest tasks she had once taken for granted.

One Life to Live deliberately allowed Viki’s recovery to unfold over months. Erika Slezak later recalled that she and the writers had discussed the responsibility of portraying stroke realistically. Letters from stroke survivors who felt the story reflected their own experiences reassured her that they were succeeding.

The storyline also found a remarkable way to reconnect Viki’s illness with her past. As Viki struggled with the effects of the stroke, Niki Smith began appearing again. When Johnny Dee later attacked an unconscious Tina at Llanfair, Niki emerged—and, unlike Viki, could stand and walk. Niki climbed the stairs and shot Johnny, saving Tina’s life. When Viki resurfaced, she had no memory of what Niki had done.

Viki eventually regained her speech and mobility and returned to her life and family. Erika later acknowledged that she wished the final stage of Viki’s recovery had been portrayed more gradually, noting that real stroke survivors can live with lasting effects for years.

The storyline was named Best Storyline by the One Life to Live Fan Club in 1991.

Viki’s husband, Clint, sees her through another difficult time in their tumultuous marriage.

“This was one of the most fun times I've played Niki, when I had the stroke,” remembers Slezak. “Niki would pop out of Viki's head, yell at her and say, 'What are you, a lazy bum?' Viki couldn't deal with her, but it was a wonderful sort of twist: 'Wait a minute, where did she come from?' Niki was kind of all over the place.”

“Freeze, Bozo!” shouts a gun-toting Niki Smith as she stops Johnny Dee from carrying away an unconscious Tina. Moments later, Niki fires the shot that kills Johnny.
